Heart&Stroke Living with StrokeTM program

The Heart&Stroke Living with StrokeTM program is a community-based support and educational program designed for stroke survivors and their caregivers to gain confidence in managing the challenges of living with stroke.   

The 8-week program allows you to develop new skills, make new friends and gain confidence in your ability to control your life. A healthcare professional leads the weekly, two-hour meetings that incorporate principles of self-management. The meetings are highly interactive, focusing on building skills, sharing experiences and lending support.

Program topics include:  

  • Understanding stroke
  • Physical changes and challenges
  • Swallowing and nutrition
  • Cognition, perception and communication
  • Emotions: Focusing on depression
  • Activities and relationships
  • Reducing the risk of stroke
  • Moving forward

Listen to stroke survivors speak frankly about the challenges they face every day, and also hear some useful advice from healthcare professionals in the field.

The Living with StrokeTM program is not intended to replace therapy or treatment. It will provide information and support to help you cope with the challenges of living with stroke.

This program is designed for stroke survivors who have completed their active rehabilitation and are living in the community.
